How Our Care Guidance Works

BestCareMatch welcomes online care-guidance inquiries from all 50 states. We help you understand care settings, prepare questions, and explore available options. Before an individual referral or provider connection, we confirm service availability and applicable state requirements, explain compensation, and obtain the required disclosures and consent. In-person consultations are normally offered in Washington. To request a visit outside Washington, email welcome@bestcarematch.com or use our contact form. Include your location, the reason for your request, and preferred timing. We review feasibility, service requirements, travel costs, and written scope before confirming a visit. Please leave sensitive records out of this first message.

What happens when you reach out

For Families & Individuals:

  • We begin by listening. Tell us about daily support needs, location, budget, routines, and the questions on your mind. We explain our placement role and agency disclosures before a referral. Clinical assessments and care decisions remain with the responsible professionals and providers.

For Care Professionals & Discharge Teams:

  • If you’re helping a family through a discharge or care change, contact us to discuss a referral. We can help organize placement questions, paperwork, and a first conversation while your team handles clinical decisions.

We discuss the care needs, preferences, budget, and professional recommendations shared during intake. The appropriate healthcare professionals and receiving provider assess clinical needs and suitability.

  • Preparing questions about Medicare, Medicaid, and the documents a program may request.
  • Addressing family and facility concerns about safety, behaviors, or chronic conditions.
  • Reviewing available official provider information and identifying details to confirm before a referral.
  • We explain what placement help includes, how information is handled, and that families pay no placement fee.
  • Providing the required agency disclosure statement per RCW 18.330
  • We help you complete the care information form required under RCW 18.330.060) gathering:
    • Medical history, diagnoses, medications, and care needs
    • We also ask about life at home, your budget, and how care may be paid for, such as Medicaid or private funds.
    • Help needed with daily tasks, memory, mental health, behavior, or a developmental disability
    • Cultural, language, or accommodation needs
    • Family and geographic preferences
  • Identify documents that may be needed, such as a power of attorney, guardianship papers, insurance, identification, or healthcare directives

We explain relevant options, available provider information, and applicable referral compensation. Families remain free to choose any provider. Our matching process covers:

  • Adult family homes, assisted, independent, or memory care communities.
  • Skilled nursing, disability, respite, and hospice care.
  • Before referral, we confirm required provider information and check official licensing and enforcement records. These checks do not certify quality, safety, compliance, or future outcomes.

We explain available information about care settings, services, and costs in plain language. Please ask whenever something is unclear.

  • Review intake information and narrow the search to providers that may meet the identified needs.
  • Review up-to-date compliance and enforcement status for each provider.
  • Contact matching providers to confirm vacancies and suitability.

We help coordinate available tours and prepare questions. Ask each provider about daily routines, services, costs, admission requirements, and any care needs it cannot meet.

  • Explain available choices and the details that need confirmation.
  • Set up tours for interested homes; accompany client/family if possible.
  • Discuss logistics, rates, and set expectations for next steps.

The receiving provider and appropriate healthcare professionals assess care needs and decide whether the setting can meet them. We help your family organize questions about move-in plans, forms, and who needs to be involved.

  • Schedule assessments by the selected home’s nurse/administrator.
  • Confirm ability to meet medical, personal, social, and safety needs.

We help organize placement paperwork and identify which documents the receiving provider or care team needs to review.

  • Facilitate agreements and admissions paperwork.
  • Confirm document requirements with the care team. The receiving provider and licensed healthcare professionals review clinical records, medication information, orders, and any required forms. We help coordinate practical questions within our non-clinical role.
  • Confirm the move-in date/time, and coordinate with the care team.

A move can bring last-minute questions. We help with the practical arrangements agreed with your family and stay in touch with the people involved.

  • Support the transition day (move-in or admission).
  • Communicate with both family and provider for any last-minute needs.

After the move, follow-up conversations provide an opportunity to share questions and discuss next steps. Contact the provider or responsible care team about care delivery or changing clinical needs.

  • Discuss follow-up timing and who to contact with questions after a move.
  • Respond to any concerns or issues, and offer ongoing support if needed.

You can share questions or concerns after a move. We’ll help you understand the next step and who is responsible for addressing it.

We review required provider information for our referral relationships. This does not replace licensing oversight or guarantee ongoing care quality.

We explain our role, applicable referral compensation, and information practices before you proceed.

  • Maintain secure records of referral, intake, recommendations, and follow-up (per RCW 18.330).
  • Report any suspected abuse or neglect as a mandated reporter.
  • Review required provider records for ongoing placement partnerships.
