1. Who We Are and What This Policy Covers

BestCareMatch LLC, doing business as BestCareMatch™ (“BestCareMatch,” “we,”
“us,” or “our”), operates https://bestcarematch.com,
https://inspectionready.bestcarematch.com, and related BestCareMatch
webpages, forms, sales pages, and online-store functions.

This Privacy Policy explains how BestCareMatch collects, uses, retains, and discloses personal information when you:

  • Use BestCareMatch Placement Services;
  • Request health-coverage or public-benefit information or application support;
  • Participate in or communicate with us about the BCM Facility Excellence
    Program™ (“FEP”), BestCareDirectory, or BCM Academy™;
  • Purchase or request support for BestCareMatch Compliance Systems;
  • Purchase or request support for Burnout C.U.R.E. Systems™;
  • Visit our websites, create an account, contact us, or place an order; or
  • Otherwise interact with BestCareMatch.

BestCareMatch provides non-clinical placement support, provider-use compliance resources, private facility-program services, and educational products. BestCareMatch is not affiliated with or endorsed by DSHS, ALTSA, RCS, or any other government agency.

2. Information We Collect

The information we collect depends on how you interact with us.

A. General Contact and Account Information

We may collect:

  • Name;
  • Email address;
  • Telephone number;
  • Mailing, billing, and shipping address;
  • Organization or facility name and professional role;
  • Account username and credentials;
  • Communications, support requests, reviews, and other information you voluntarily provide.

B. Placement Services Information

When a person requests Placement Services, BestCareMatch may collect information about the prospective resident, client, authorized representative, and relevant contacts. This may include:

  • Contact and representative information;
  • The type and location of supportive housing or care requested;
  • Current living situation and geographic preferences;
  • Recent medical history relevant to placement;
  • Known medications and medication-management needs;
  • Diagnoses, health concerns, symptoms, behaviors, and special-care needs;
  • Mental illness, dementia, or developmental-disability information, if applicable;
  • Assistance needed with activities of daily living;
  • Language, cultural, accessibility, activity, food, sleep, and routine preferences;
  • Basic financial information relevant to placement options, including payment source, long-term-care insurance, Medicaid, or financial-assistance availability;
  • Provider options considered, referrals made, disclosures, authorizations, acknowledgments, communications, and placement outcome information; and
  • Information required to verify provider licensing, capabilities, services, and enforcement status.

BestCareMatch obtains placement information from the client or prospective resident whenever practicable and may also obtain information from an authorized representative, family member, caregiver, health professional, social worker, discharge planner, case manager, provider, or official government source.

Placement-related health information is handled under applicable Washington law. Please also read our separate Consumer Health Data Privacy Policy at:

C. Facility Excellence Program, BestCareDirectory, and BCM Academy Information

For FEP, BestCareDirectory, or BCM Academy inquiries, participation, visits,
enrollment, or documentation, we may collect:

  • Facility and business contact information;
  • Owner, entity-representative, administrator, or staff contact details;
  • Facility address, license type, license number, capacity, services, payment sources, and other provider-profile information;
  • Scheduling, contract, payment, and program communications;
  • Academy enrollment, access, participation, and completion information, when
    applicable;
  • Provider-submitted documents and information;
  • Dated visit records, observations, photographs, videos, or 360-degree visual documentation created or approved for the program; and
  • Information necessary to prepare an approved BestCareDirectory profile or time-limited FEP recognition, when applicable.

Facilities must not provide resident records, medical documents, private resident information, or images identifying a resident unless disclosure is legally permitted and specifically authorized. Public profiles use approved business information and approved visuals; private FEP records are not automatically made public.

FEP, BestCareDirectory, and BCM Academy are private BestCareMatch programs or
services. They are not a DSHS inspection, approval, accreditation,
certification, or substitute for government licensing or oversight.

D. Compliance Systems and Burnout C.U.R.E. Systems™

For BestCareMatch Compliance Systems and Burnout C.U.R.E. Systems™, we may collect:

  • Customer and purchaser contact information;
  • Product selected, edition, quantity, price, and order status;
  • Shipping, delivery, download, or access information;
  • Product-registration or license information, if applicable;
  • Customer-service, correction, replacement, or refund communications; and
  • Voluntary feedback or reviews.

These Systems are educational or provider-use resources and are not a channel for submitting resident records, patient information, medical records, incident reports, personnel records, or other sensitive facility documents. Please do not include such information in an order note, product-support request, review, or email.

Burnout C.U.R.E. Systems™ are educational and implementation resources. They do not provide medical diagnosis, mental-health treatment, emergency care, or individualized clinical services.

E. Health-Coverage and Public-Benefit Support

If you request Medicare, Medicaid, Affordable Care Act Marketplace, or other
health-coverage or public-benefit information or application support, we may
collect information reasonably necessary to understand and respond to the
request. Depending on the service requested, this may include:

  • Name, contact information, age or date of birth, residence, household
    information, and preferred language;
  • Current coverage, eligibility, enrollment, renewal, or application
    information;
  • Financial or household information relevant to eligibility;
  • Information about an authorized representative;
  • Application or case reference information; and
  • Communications and documents you voluntarily provide for the requested
    assistance.

Do not send Social Security numbers, Medicare or Medicaid identifiers, full
insurance identification numbers, immigration documents, financial-account
information, or health records through a general contact form, ordinary email,
text message, product order note, or public review. BestCareMatch will provide
an appropriate method if sensitive information is necessary for a requested
service.

Government programs, benefit exchanges, insurers, enrollment platforms, and
other entities involved in an application or enrollment process have their own
privacy notices and legal duties. This Privacy Policy does not replace those
notices.

F. Orders and WooCommerce

When you place an order, we may collect:

  • Name, email address, telephone number, and billing and shipping addresses;
  • Products purchased, quantity, price, discounts, taxes, shipping charges, and total;
  • Order, payment, shipping, cancellation, return, and refund status;
  • Account information, if an account is created; and
  • IP address, device, browser, and fraud-prevention information associated with the transaction.

Our store uses WooCommerce. Information is used to process payment, fulfill and ship orders, provide customer service, issue refunds, prevent fraud, and maintain legally required business records.

G. Payments

Payments may be processed through WooPayments and its payment-processing partners, including Stripe. Depending on availability, eligibility, and your selection, Klarna or Afterpay may also process payment or financing information.

Payment providers receive information necessary to authorize and process a transaction, prevent fraud, manage disputes, and comply with financial-service requirements. BestCareMatch does not receive or store your complete payment-card number.

H. Website, Device, and Technical Information

We may collect:

  • IP address;
  • Browser and device type;
  • Referring page or campaign;
  • Pages viewed and links selected;
  • Date, time, and duration of a visit;
  • Cookie or similar identifiers;
  • Shopping-cart, checkout, and account interactions; and
  • Security, error, diagnostic, and fraud-prevention information.

3. How We Use Personal Information

We may use personal information to:

  • Respond to inquiries and provide requested services;
  • Conduct the placement intake and referral process;
  • Identify provider options that appear capable of meeting a prospective resident’s stated needs;
  • Communicate with a client, authorized representative, professional, or selected provider;
  • Provide requested health-coverage or public-benefit information or
    application support and communicate with authorized programs, platforms,
    insurers, or representatives when necessary and permitted;
  • Prepare and retain required placement disclosures, acknowledgments, authorizations, referral records, and provider-verification records;
  • Schedule, perform, document, and administer FEP, BestCareDirectory, and BCM
    Academy services;
  • Prepare approved provider business profiles and visual documentation;
  • Process, fulfill, ship, cancel, return, replace, or refund orders;
  • Calculate taxes and shipping charges;
  • Send transactional, service, shipping, and customer-support communications;
  • Administer accounts and product access;
  • Protect consumers, residents, providers, BestCareMatch, and the public from fraud, misuse, security threats, or unlawful activity;
  • Comply with mandated-reporting, accounting, tax, contractual, legal, and regulatory duties;
  • Establish, exercise, or defend legal claims;
  • Maintain and improve website functionality and accessibility; and
  • Measure the performance of non-sensitive product advertising when appropriate tracking is enabled.

4. Placement Authorization and Referral Disclosures

BestCareMatch may work with both the client and a care-services or supportive-housing provider in the same placement transaction. Before making a covered referral, BestCareMatch uses the disclosure, acknowledgment, intake, authorization, and recordkeeping processes required by Washington law.

BestCareMatch may give a client the names of providers that may meet the identified needs. BestCareMatch will not give a provider the client’s name or disclose confidential health-care information without the authorization required for that referral or disclosure, except when disclosure is otherwise legally required or permitted.

The Privacy Policy itself is not a referral authorization. A separate authorization or consent will be used when required.

5. FEP, Referral, and Product Independence

FEP participation, BestCareDirectory participation, Academy participation, purchases of Compliance Systems, and purchases of Burnout C.U.R.E. Systems™ are separate from placement-referral decisions.

Payment does not buy a referral, preferred listing, favorable FEP finding, government approval, or inspection outcome. No purchase or program participation guarantees referral volume, occupancy, placement, regulatory results, business results, or a favorable government action.

6. When We Disclose Information

We disclose information only as reasonably necessary for the purposes described in this policy, as authorized, or as permitted or required by law.

A. Placement Providers and Authorized Participants

For a requested placement service, we may disclose limited client information to:

  • A care-services or supportive-housing provider selected for referral;
  • The client, prospective resident, or authorized representative;
  • A professional assisting the client, such as a social worker, discharge planner, case manager, nurse, or other authorized professional; or
  • Another person specifically authorized by the client or legally authorized representative.

We limit placement disclosures to information reasonably relevant to evaluating or providing the requested service.

B. FEP, Directory, and Academy Information

We may publish provider business information, approved facility descriptions,
and approved visuals in an FEP or BestCareDirectory profile. We may use
enrollment, participation, and completion information to administer BCM Academy
services. We do not intentionally publish resident records or private resident
health information.

C. Payment, Financing, Shipping, and Fulfillment Providers

We may disclose order and transaction information to payment processors, financing providers, tax services, postal services, shipping carriers, fulfillment providers, and related vendors as necessary to process, fulfill, deliver, track, cancel, return, or refund an order.

D. Health-Coverage and Public-Benefit Participants

At your request or with appropriate authorization, we may disclose information
reasonably necessary for application or enrollment support to government
programs, benefit exchanges, insurers, authorized enrollment platforms, or
other persons participating in the requested process. We do not use sensitive
application information for unrelated advertising.

E. Technology and Business-Service Providers

We may disclose information to vendors that provide website hosting, e-commerce, security, backup, email, communications, data storage, customer support, analytics, fraud prevention, accounting, insurance, or professional services. These providers may use information only for the contracted or legally permitted purpose.

F. Legal, Safety, and Regulatory Disclosures

We may preserve or disclose information when reasonably necessary to:

  • Comply with a law, court order, subpoena, regulatory request, or legal process;
  • Meet mandated-reporting duties;
  • Report or respond to suspected abuse, neglect, abandonment, exploitation, fraud, or other unlawful conduct;
  • Protect the rights, safety, and security of a consumer, resident, provider, BestCareMatch, or another person; or
  • Establish, exercise, or defend legal claims.

G. Business Transfers

Information may be transferred as part of a merger, acquisition, financing, reorganization, bankruptcy, or sale of business assets, subject to applicable law and continued protection of the information.

7. No Data Brokerage, Lead Farming, or Advertising Use of Client Records

BestCareMatch does not sell client records, placement health information, or
consumer health data as a lead list, data-broker product, or advertising
audience. BestCareMatch does not sell ordinary website-visitor or customer
information for monetary payment.

BestCareMatch may receive a provider-paid referral fee for placement services
when permitted and disclosed under Washington law. Client information is
disclosed only as authorized or legally permitted and as reasonably necessary
to provide the requested referral service. Provider payment never authorizes
unrelated marketing, resale, redistribution, or advertising use of client
information.

Washington’s My Health My Data Act defines a “sale” of consumer health data
broadly. If a contemplated disclosure is legally classified as a sale under
that Act and is not exempt, BestCareMatch will not make the disclosure unless
it first obtains the separate, valid authorization required by applicable law.

We do not disclose placement health information to advertising platforms.

8. Cookies and Similar Technologies

Our websites may use cookies and similar technologies to operate:

  • Shopping carts and checkout;
  • Customer accounts and login functions;
  • Security and fraud-prevention controls;
  • Website preferences;
  • Site performance and error diagnosis; and
  • Advertising measurement on eligible, non-sensitive product pages when enabled.

You can control or delete cookies through your browser settings. Blocking necessary cookies may prevent the cart, checkout, account, login, or other website functions from working properly.

If comments are enabled, you may choose to save your name, email address, and website in cookies for convenience. WordPress login and administrative cookies may also be used for authorized users.

9. Analytics and Advertising

When enabled on appropriate, non-sensitive Compliance System sales and checkout
pages, BestCareMatch may use advertising-measurement tools such as the Meta
Pixel or Meta Conversions API. These tools may receive limited website-event
information, such as page visits, product views, checkout activity, purchases,
browser and device information, IP address, cookie identifiers, and transaction
value.

Advertising tracking must not be used on Placement Services pages or intake
forms, health-coverage or public-benefit application pages, confidential-health-
information authorizations, privacy-request pages, Consumer Health Data Privacy
pages, or other pages that collect or reveal a person’s care needs, health
information, financial eligibility, government identifiers, or benefits
information. BestCareMatch does not intentionally send resident records,
placement intake responses, medical information, diagnoses, medications,
benefit-application information, or other sensitive information to Meta.

Meta tracking must remain off Burnout C.U.R.E. Systems™ product, assessment,
reflection, and support pages unless a documented legal, privacy, and Meta-terms
review confirms that the particular page and event cannot reveal or imply
health or wellbeing information. A shared WooCommerce checkout must be
configured so that Burnout C.U.R.E. Systems™ product names, content identifiers,
URLs, order notes, and customer-support information are not transmitted to an
advertising platform.

If BestCareMatch cannot reliably separate eligible Compliance System events
from restricted Placement Services, benefits-support, or Burnout C.U.R.E.
Systems™ events in a shared WooCommerce checkout, advertising tracking must
remain off the shared checkout and purchase-confirmation pages.

Meta processes information under the Meta Privacy Policy and Meta Business Tools Terms.

10. Comments, Reviews, Media, and Embedded Content

If comments or reviews are enabled, we may collect the information entered, together with IP address and browser user-agent information, to assist with spam detection, moderation, and security.

An anonymized value created from an email address may be sent to Gravatar to determine whether the commenter uses that service. After approval, an associated profile image may be publicly visible. Gravatar is covered by the Automattic Privacy Policy.

If a website feature permits image uploads, avoid uploading files containing embedded location data such as EXIF GPS information. Visitors may be able to download public images and extract embedded information.

Our websites may include embedded videos, images, articles, maps, payment tools, or other third-party content. Embedded content may behave as though you visited the third-party website directly. Those providers may collect information, use cookies, or track interactions under their own privacy policies.

11. How Long We Retain Information

We retain personal information only as long as reasonably necessary for the purposes described in this policy and as required or permitted by law.

  • Placement records: Washington referral-agency records covered by chapter 18.330 RCW are retained for six years.
  • FEP and provider records: Retained for the duration of the program relationship and afterward as reasonably necessary for contracts, visit documentation, profile history, payment records, disputes, legal compliance, and program integrity.
  • Orders and transactions: Retained as reasonably necessary for fulfillment, customer service, accounting, taxes, refunds, warranties, chargebacks, fraud prevention, and legal compliance.
  • Accounts and communications: Retained while an account or service relationship remains active and afterward as reasonably necessary for security, support, recordkeeping, and legal obligations.
  • Cookies and analytics: Retained according to the applicable cookie, configuration, and service-provider settings.

Legal retention obligations may limit deletion of some records. Payment processors, shipping providers, and other third parties retain information according to their own policies and legal duties.

12. Your Privacy Choices and Rights

Depending on the information and applicable law, you may request that BestCareMatch:

  • Confirm whether we maintain personal information about you;
  • Provide access to or a copy of certain information;
  • Correct inaccurate information;
  • Delete information that we are not legally required or permitted to retain;
  • Withdraw a consent or authorization for future collection or disclosure;
  • Stop certain communications; or
  • Review a privacy decision or complaint.

Clients also have access, upon request, to BestCareMatch referral-agency records concerning them as provided by Washington law.

To begin a privacy request, email welcome@bestcarematch.com with the subject line Privacy Request. Do not include medical details, passwords, complete payment-card numbers, or other sensitive information in the initial email. We may provide a more secure method and request information reasonably necessary to verify your identity and authority.

We may retain information necessary to comply with placement-record, accounting, tax, fraud-prevention, security, contractual, or other legal obligations.

13. Security

We use reasonable administrative, technical, and organizational safeguards designed to protect personal information. Access to placement and consumer health information is limited to persons who need it for an authorized purpose or to provide a requested service.

No website, email, transmission method, or storage system can be guaranteed completely secure. Please do not send unnecessary sensitive information through ordinary email, public comments, reviews, order notes, or general contact forms.

14. Children’s Privacy

Our websites, Placement Services, FEP, Compliance Systems, and Burnout C.U.R.E. Systems™ are intended for adults and professional users. We do not knowingly collect personal information online directly from children under 13. If you believe a child has submitted personal information, contact us so we can review and address the matter.

15. Password Resets

If you request a password reset, information such as your IP address may be included in or associated with the password-reset process for security purposes.

16. External Websites

Our websites may link to government agencies, service providers, payment providers, carriers, and other third-party websites. BestCareMatch does not control and is not responsible for the privacy practices of third-party websites.

17. Changes to This Policy

We may update this Privacy Policy when our services, products, websites, vendors, or legal obligations change. The revised policy will display an updated effective date. When required, we will provide additional notice or obtain consent before using information for a materially different purpose.
